Lifting The Lid On Blepharoplasty Surgery

As a Consultant Ophthalmic Plastic, Reconstructive & Cosmetic Surgeon, I always notice there's a point at which patients stop talking about “looking tired” and start describing something more specific. They tell me their eyelids feel heavy, that their makeup no longer sits properly, or they are lifting their brows to see more clearly, particularly by the end of the day. This is often when we start talking about blepharoplasty surgery.

Blepharoplasty is a surgical procedure designed to remove excess skin, and in some cases adjust fat, from the eyelids. This can involve the upper eyelids, the lower eyelids, or occasionally both together, depending on the individual. In my practice, this most often relates to the upper eyelids.
The intention is not to change the eye itself, but to restore a cleaner, more defined eyelid contour by removing redundant skin and re-establishing the natural crease. Done properly, it should not look like surgery, rather it should simply look as though the eye area has been refreshed and clarified.
Why Consider Blepharoplasty?
The reasons people consider blepharoplasty tend to fall into two broad categories, although there is often overlap. The first is aesthetic. The skin of the eyelid is exceptionally thin and, over time, loses elasticity. At the same time, the brow may descend slightly. Together, these changes can lead to hooding of the upper lid, a loss of definition, and a more closed or fatigued appearance. For many patients, this is less about ageing in general and more about a specific sense that their eyes no longer reflect how they feel.
The second reason is functional. In some cases, the excess skin becomes significant enough to interfere with the upper field of vision, particularly laterally. Patients describe a physical sense of weight in the eyelids, or a need to lift their brows in order to see more clearly. In these situations, blepharoplasty improves both comfort and visual function.
At my clinic in Worcester, the procedure is usually performed under local anaesthetic as a day case. The incision is placed within the natural eyelid crease, which allows it to be well concealed once healed. The amount of skin to be removed is carefully measured, as precision here is essential. Too little, and the result is underwhelming. Too much, and the eye can appear overcorrected or unnatural.

Excess skin is removed, and where appropriate, small adjustments may be made to the underlying fat. The incision is then closed with very fine sutures. The entire procedure typically takes under an hour.
The key to a good outcome is restraint. The aim is to preserve normal anatomy and avoid a hollowed or operated appearance. Most people will not be able to identify that surgery has taken place. They will simply notice that you look more open, or more rested.

What About Recovery?
Recovery is generally straightforward. There is usually some swelling and bruising in the first week, which settles steadily over the following days. Sutures are removed at around one week, and most patients feel comfortable returning to normal social activities within ten to fourteen days. The scar itself sits within the natural crease and typically heals very well.
One aspect that is often underestimated is the effect on the ocular surface. Following eyelid surgery, it is quite normal for the tear film to be temporarily disrupted, which can lead to dryness or a sensation of irritation. This is not unexpected, but it is something that should be actively managed.
Post-Surgery Aftercare
One area I place particular emphasis on is what happens after surgery. Blepharoplasty is often described as a relatively straightforward procedure, and technically it is. But the quality of the result is shaped just as much by how the tissues heal, how comfortable the eyes feel, and how well the ocular surface is supported in those early days. For that reason, I offer a more considered approach to aftercare.

At around seven to ten days following surgery, once the initial swelling has begun to settle, our patients then return to clinic for a full ocular surface assessment. This allows me to look carefully at tear film stability and identify any early signs of dryness or irritation, which are not uncommon after eyelid surgery. Addressing this early makes a noticeable difference, both in comfort and in how the eyes recover.

It stimulates mitochondrial activity, which in turn helps reduce inflammation and improves local circulation.
